First office call (FOC) – in office or Telemed visit for medical concern/illness care: for a SPECIFIC medical concern/s (Copay OR $75 day of service if have not met annual deduct.). First visit will be focused on collecting info and understanding your overall health and where the chief complaint fits into your bigger wellness picture. Please bring: list/bag of current medications & supplements.
Return office visit (ROC) – in office or Telemed visit for medical concern/illness care. This visit is for SPECIFIC medical concern/s that fall outside of an annual preventative medicine exam which include physical exam, labwork, preventative screening exams. If you have met your deductible for the year, you will be charged your copay. If not, $75 payment is due.
